Rebound Policies Regarding COVID-19:
Do not come if you are feeling unwell, please reschedule/cancel
- Upon arrival, please identify yourself as a clinic client to the Goodlife Greeter (do not line up), they will let you in
- No Walk-In patients will be accepted at this time, please call us or book online
- You will be contacted the day before to complete a mandatory COVID-19 screening
- Please come alone to your appointments (Exceptions: infants, guardian to young patient, support person for disabled patients)
- Please do not arrive earlier/later than 5 minutes, please wait outside the building if you are early (for example, in your car)
- Sanitize your hands on arrival, and when leaving
- Access to the gym is not permitted
- Contactless payments are required, paperless transactions are preferred

All patients, visitors and staff must answer NO to ALL the following questions to be allowed at the clinic:
1. Did the person have close contact with anyone with acute respiratory Illness or travelled outside of Ontario in the past 14 days?
2. Does the person have a confirmed case of COVID-19 or had close contact with a confirmed case of COVID-19?
3. Does the person have any of the following symptoms:
•Fever
•New onset of cough
•Worsening chronic cough
•Shortness of breath
•Difficulty breathing
•Sore throat
•Difficulty swallowing
•Decrease or loss of sense of taste or smell • Chills
• Headaches
•Unexplained fatigue/malaise/muscle aches (myalgias) •Nausea/vomiting, diarrhea, abdominal pain
•Pink eye (conjunctivitis)
•Runny nose/nasal congestion without other known cause
4. If the person is 70 years of age or older, are they experiencing any of the following symptoms: delirium, unexplained or increased number of falls, acute functional decline, or worsening of chronic conditions?
If you have answered yes to any of these questions, you are advised to:
• not attend in person at the office for at least 14 days;
• complete the Ontario Government’s self-assessment; and
• contact an appropriate authority such as their family physician, local medical officer of health or Telehealth Ontario.
